How HRA Works
As a complement to Matria’s disease management services, we offer personalized Health Risk Assessment (HRA). This online question-and-answer tool for your employees helps you identify potential candidates for disease management, wellness and prevention programs more quickly than through a disease management program.
Intended for your entire population, our HRA gives employees/members a convenient and private way to understand their current and potential future health needs. It guides employees through their past medical history and helps them:
- Identify specific health risks
- Establish steps for improving health and reducing health risks
- Develop customized information and determine actions to mitigate health risks
- Assess their readiness-to-change
- Monitor their progress towards healthy goals
By offering HRA as a benefit to your population, your company not only shows concern for its employees, but also helps to increase productivity and reduce healthcare costs by promoting an overall healthier workforce.
For the Employee:
- Employees access the Health Risk Assessment via a Web site or a paper form that can be scanned.
- They answer a series of questions about their family history, general health measures such as weight, blood pressure, cholesterol levels and lifestyle behaviors.
- Upon completion of the assessment, they receive a comprehensive, confidential report of the assessment results, featuring their overall health outlook.
- Next, they complete the readiness-to-change questions, submit their answers and receive a detailed report suggesting concrete steps they can take to start improving their health.
- Patients are given a wallet card to collect and record information from doctor visits, lab tests, weight and height.
For the Employer:
- Employers receive the following reports:
- Activity reports showing who and how many people actually completed the HRA
- Aggregate population reports showing population statistics and percentage
- If an employee is identified with a specific health issue, they are automatically contacted with information about relevant health programs.
